How to make changes to your licence

This page contains information on changing, updating or closing your HTA licence including changes to the Designated Individual (DI), Licence Holder (LH) or Corporate Licence Holder contact (CLHc).
 
 
 It is a legal requirement to inform the HTA of any changes to your licence or contact details. This ensures we can reach the DI and Licence Holder LH or Corporate Licence Holder contact CLHc. Please email updates and variation requests to licensing@hta.gov.uk.

An application to vary a licence can be made by either the DI or the LH / CLHc, with the exception of a change of DI which needs to be submitted by the LH / CLHc. All applications to vary a licence are subject to evaluation by HTA before approval.

Change of Designated Individual

Please complete this form if you need to replace the Designated Individual (DI) on an existing HTA licence.

Change of Corporate Licence Holder

If you need to replace the Corporate Licence Holder with another corporate body, or replace an individual Licence Holder with a Corporate Licence Holder (and associated contact) you will need to complete one of the forms below.

Change of Licence Holder

If you need to replace an Individual Licence Holder with another you will need to download and complete one of the forms below.
 

Change of Person Designated (PD)/ODT Named Individual (NI)

In order to update your list of PDs / NIs please download and complete the below form.  

The DI or LH / CLHc will need to submit this request to licensing@hta.gov.uk

Changes to licensed premises

If you need to make any substantial changes to an existing licensed premises (for example an extension to the premises) or will be operating out of new premises, you will need to download and complete one of the forms below.

Changes to licensable activities

Should you need to change your licensable activities, please send details to licensing@hta.gov.uk for assessment.
